Care Connection is a program that is conducted by the Visiting Nurse Association of Chittenden and Grand Isle Counties. It includes a range of personal and medical home health care services. The program offers geriatric and respite care management and private nursing services. It provides personal care assistance with bathing and dressing, feeding, meal preparation, light housekeeping and shopping. Care Connection includes medication management and transportation services. The program operates with a staff of home care providers, certified nursing assistants, nurses, geriatric care specialists and other licensed medical professionals. In addition, it accepts insurance from various carriers.

Visiting Nurse Association is a 104-year-old home health agency caring for people of all ages groups.
