The decision of whether or not to receive hospice care is always the patient’s.  We realize this is often a difficult decision, and we care help patients and families work through this sensitive process.  When considering whether hospice care is appropriate, understand Albany Community Hospice is an option for people facing any disease for which a cure is no longer possible.

The best time to learn about hospice is before you need it.

When the goal of treatment begins to shift from curing the illness to providing comfort, it is time to consider hospice.  Hospice is not about dying – it is about living every day until you die.

Albany Community Hospice can provide services to patients and families for months, not just for their final hours or days.  By contacting hospice eary in the diagnosis, the patient and family are able to spend precious time together.

Our hospice team emphasizes emotional, spiritual and physical well-being.  We determine each patient’s needs and then work with the patient and family to provide the best care.  Our team provides compassionate care and support for patients in their own homes, nursing and assisted living facilities, helping to relieve or decrease pain and other symptoms.

The team approach works to maximize comfort for the terminally ill person and to help support the family members and loved ones.

Our interdisciplinary hospice team works with patients and families to create a plan of care, making sure that everyone receives all of the care, support and information needed.

The hospice team is made up of board certified physicians, registered nurses, social workers, home health aides, chaplains, bereavement counselors, and volunteers.
